I would like to schedule an appointment. How do I proceed?

- DelLisa Eddington, N.P. is currently seeing patients Tuesday, Wednesday, and Thursday (remote appointments, face-to-face scheduled appointments, and afternoon walk-ins accepted as long as you do not have a fever or COVID-19 symptoms).
- Annette Davis, C.N. is not currently accepting new patients. Scheduled mentoring appointments may be available during the work week, when scheduled five to seven days in advance.
- Jean Bokelmann, M.D. has retired from seeing EIMC patients. She will still be involved with EIMC giving classes and seminars.
- Dr. Jean-Claude Lapraz is currently dedicating his time to writing and training physicians so that more people can have access to endobiogenic care throughout the world. He urges you to continue your endobiogenic care in the capable hands of the EIMC staff.
I am not able to travel to the clinic at this time. Can I have a distance evaluation?
Yes. While a face-to-face visit is preferred, we understand that travel is not always medically feasible. Before your labs are drawn, you will be required to sign a declaration of informed consent which includes authorization for us to send critical information such as lab alerts to your primary care provider. We highly encourage a face-to-face visit as soon you are able.
What do I need to bring with me to my appointment?
Please bring (or send in advance) the following:
- A copy of your insurance card (if applicable).
- Choose an appropriate health history from the list below:

What labs are required?
Most patients who desire an endobiogenic evaluation require a Rank 3 labs. The charge for this extensive lab work (over $700.00 value!) is included in your endobiogenic evaluation fee. We also recommend a PSA for men 40 and over for an extra $30.
When should I have my labs run?
Please have your labs drawn at least 2 weeks before your appointment so that your Biology of Functions report can be ready for review at your appointment.
*Note: A 12 hour fast is required prior to your lab draw (you may drink water and take any required medications).

If you live out of state, a lab order can also be faxed to your local LabCorp. Please see http://www.labcorp.com/psc/index.html to locate the LabCorp draw center nearest you. Call or e-mail us with the phone number of the draw center of your choice, provide us with your payment information, and we will fax your lab order.
Can I be seen at EIMC without Biology of Functions Labs?
Yes. While we consider the Biology of Functions to be a very useful diagnostic tool, it is not an absolute requirement. We also offer a “basic evaluation” which looks for endocrine and nervous system signs based on your history, symptoms, and physical exam.
If I have questions regarding my herbs, supplements, dietary or lifestyle recommendations, who can I ask?
Schedule an appointment to speak with DelLisa Eddington is our Nurse Practioner that also specializes in Endobiogeny, and manual manipulation. She will be able to answer questions about your herbs, supplements, dietary and lifestyle recommendations. We remind you to consult your primary care physician for pharmaceutical needs, ongoing health maintenance, prevention, evaluation and management of any acute minor or serious medical problems. Do you accept insurance?
Yes, physician’s visits by DelLisa Eddington can be billed to most health insurance plans. (Sorry, we are not able to accept Medicare or Medicaid, however we do offer a cash discount). If you choose to have Biology of Functions labs (highly encouraged, but not required), there is a $350.00 out of pocket fee which covers the following:
- Extensive lab work performed at your local LabCorp including: CBC, TSH, LDH, CPK, Osteocalcin, Alkaline Phosphatase with Hepatic, Bone, Intestinal Isoenzymes, ESR, Lipid Panel (over $700.00 value!)
- We also recommend the PSA test for men 40 and over for $30 extra
- Processing your lab results through the Biology of Functions software (summary printout included)
- Recommendations for supplements, dietary and lifestyle changes by our Endobiogeny Team

Do you accept out of state patients?
Yes. Below is some information to help you with airline and hotel reservations.

Nearest international airport (approximately 2.5 hours driving distance): Salt Lake City, UT (SLC).
Rental cars or a shuttle is available from SLC to PIH: Salt Lake Express Express: www.saltlakeexpress.com
Recommended hotels: Marriott Towne Place Suites (208) 478-7000; Ameritel Inn (800) 600-6001; Red Lion Inn (800) 527-5202; Holiday Inn (800) 200-8944. Red Lion Inn and Holiday Inn both offer a free 24 hour airport shuttle. Ameritel offers a free Airport shuttle 8:00 A.M. – 7:00 P.M.
Driving directions: From I15, exit #63 Portneuf Area. Turn left (North) onto South 5th Ave. Proceed North on South 5th for approximately 1.5 miles. We are on the right (East Side of the road) in a large tan building with a green roof.

Physical Exam
- Physical exams performed by DelLisa Eddington or Dr. Wheeler can be billed to most insurance carriers upon request. (Sorry we cannot accept Medicare or Medicaid, however we do offer a cash discount). Applicable patient co-pays and deductibles are due at the time of service. *Any portion of the fee not paid by your insurance carrier is your responsibility.

Endobiogenic Evaluation
- (highly encouraged, but not required.)
- $350.00 (prepaid out of pocket) ($375 with PSA for men 40 and over)
- This fee includes the following:

- Processing your lab results through the Biology of Functions software (summary printout included).
- Recommendations for supplements, dietary and lifestyle changes by our Endobiogeny Team
- The physical exam fee is NOT included (see above).

Remote Evaluation: $400 with Annette, $435 with DelLisa Prepaid out of pocket (includes labs)
- While a face-to-face visit is preferred, we understand that travel is not always medically feasible. We therefore offer preliminary distance evaluations as well as follow-up remote evaluations. Before your labs are drawn, you will be required to sign a declaration of informed consent which includes authorization for us to send critical information such as lab alerts to your primary care provider. We suggest that you schedule a face-to-face visit as soon you are able. The fee includes the following:
- Extensive lab work performed at your local LabCorp including: CBC, TSH, LDH, CPK, Osteocalcin, Alkaline
Phosphatase with Hepatic, Bone, Intestinal Isoenzymes, ESR, Lipid Panel; $425.00 with Annette, $460 with
DelLisa, with PSA for men 40 and over (over $700.00 value!) - Processing your lab results through the French Biology of Functions software (summary printout included).
- Recommendations for supplements, dietary and lifestyle changes by our Endobiogeny team.
- A telephone appointment to review your results.
- Labs. Please call for a lab order at least 2 weeks prior to your appointment so that your Biology of Functions report can be ready for review. A 12 hour fast is required prior to your draw. If applicable, any additional labs ordered will incur a separate fee. A 12 hour fast is required prior to your draw (you can drink water and take any required medications).

NEW! Families’s Annual (Preventive Health) Exams
- EIMC now offers Families’s Health Exams performed by our caring providers in a relaxed, comfortable environment. The fee can be billed to most insurance carriers upon request. Applicable patient co-pays and deductibles are due at the time of service. Any portion of the fee not paid by your insurance carrier is your responsibility.
* Please note that the Endobiogenic Evaluation is billed as a separate charge from the physical exam. It is a prepaid, non-refundable, out of pocket charge that cannot be billed to your insurance provider. Fees are subject to change at any time.
